The C and O Employees’ Hospital Association (“COEHA”) has a contract with Medicare to pay as primary on certain Part B services under contract H4906 (Health Care Prepayment Plan) for CREDENTIALED PROVIDERS ONLY, which is why you see us listed as an “HMO”. We are not a standard Medicare Advantage Plan (HMO)!!  The Bill Code 1 at the bottom of your screen means that the fiscal intermediary (CMS) will process all claims (see attached email from CMS explaining the Bill Code).

If you have not been credentialed through COEHA, then you need to file your claims with the member’s MBI through Railroad Medicare at the address below.  Railroad Medicare will pay the claims. After Medicare pays their 80% of the claim then it crosses over to us from COBC (Coordination of Benefits Contractor) and we will pay the remaining deductible and coinsurance amount due, so there is no need to file the claim to us as supplemental once Medicare pays.
